Paragon Guides Important Forms

Once you have signed up for a multi-day trip with Paragon, we will be sending you the forms needed for your trip.  Paragon uses a digital form format from EchoSign, or we can send you the PDF or paper versions.  If you need the form again, choose from those listed below. For each trip participant, please complete and submit one of each of the following forms:
Summer:

• Summer Reservation Form
• PG Medical History
• PG Liability Waiver
• 10th Mountain Division Hut Association Liability Waiver
  • Adult (18 years of age or older)
  • Minor (under 18 years of age)

Winter:

• Winter Reservation Form
• PG Medical History
• PG Liability Waiver
• 10th Mountain Division Hut Association Liability Waiver (select one)
  • Adult (18 years of age or older)
  • Minor (under 18 years of age)
